We invite you to complete the Health History Form and the Consent for Use and Disclosure of Health Information Form before you arrive at the office for a new patient visit. For your convenience, you can complete and submit these forms online by selecting the “Online Submission Forms” links or select the “Download / Print Forms” to manually complete the forms at your leisure and present them at the new patient’s scheduled appointment. We also offer the option of arriving 15 minutes before your child’s first appointment in order to complete all the forms in our office. Authorization Forms
If a parent or legal guardian is unable to bring their child to a scheduled appointment after their initial New Patient visit, they may complete an Authorization for Alternate Adult to Accompany Child Form online and submit a signed copy to our office before any scheduled appointment where the alternate adult will be the person responsible for the patient. Please note that these authorizations may only be submitted by a parent or legal guardian after they have completed all New Patient paperwork and accompanied their child on their first visit.
